In order to simplify your licence renewal process and improve our understanding of how you're using JACK, please complete the following questionnaire.

Be sure to submit to us by 29 June to prevent your JACK licence expiring.  

A fully completed form fulfils your annual requirement for licence renewal and once received, we will renew your JACK Academic Licence in July, as per the licence agreement.
